What happens after active treatment changes or ends?

Aftercare should be agreed before a medicine, intensive visit schedule or structured programme changes. It includes the prescriber's transition instructions, a minimum routine, maintenance reviews and an early-action threshold for persistent regain or returning health concerns.

  1. Plan the transition

    Discuss why treatment is changing and follow the prescriber's instructions for stopping, switching or restarting medicine.

  2. Keep the minimum routine

    Protect a realistic meal, movement, strength and sleep pattern for normal and difficult weeks.

  3. Schedule maintenance

    Continue appropriate reviews of weight trend, nutrition, strength, mood and relevant health markers.

  4. Act before a major setback

    Use an agreed contact range when appetite, symptoms, glucose or weight changes persist.
